These are the criteria a clinician applies to your answers. They are published here, in full, before you create an account — including the ones that will rule you out.

Weight-related conditions
- Type 2 diabetes
- PrediabetesA fasting glucose or A1c in the prediabetes range
- High blood pressureDiagnosed, or treated with medication
- High cholesterol or triglycerides
- Obstructive sleep apnoea
- Heart diseaseIncluding a previous heart attack, stroke, or stent
- Fatty liver diseaseAlso called NAFLD or MASLD
- Osteoarthritis affecting weight-bearing joints
- Polycystic ovary syndrome (PCOS)
Hard stops
What rules treatment out entirely
Each of these stops the questionnaire. None of them is a reason to be embarrassed, and none of them costs you anything — the funnel ends before any card details are asked for.
- GLP-1 medications are not safe for people with a personal or family history of medullary thyroid cancer.
- GLP-1 medications are not safe for people with Multiple Endocrine Neoplasia syndrome type 2.
- GLP-1 medications are not recommended during pregnancy, while breastfeeding, or while trying to conceive.
- Because you have had pancreatitis, GLP-1 medications are not appropriate without in-person specialist care.
- Our weight-management programme is not designed for people with type 1 diabetes.
- You have reported a hypersensitivity reaction to a GLP-1 medication.
- You must be 18 or older to use this service.
- GLP-1 medications for weight management are indicated at a BMI of 27 or above.
- At a BMI between 27 and 30, GLP-1 medications are indicated only alongside a weight-related health condition such as high blood pressure, type 2 diabetes, high cholesterol, or sleep apnoea.
- Your responses suggest a disordered-eating pattern that deserves dedicated care before starting weight-loss medication.
- GLP-1 medications slow stomach emptying and are not safe with severe gastroparesis.

These do not rule treatment out. They route your file to a clinician with the flag attached.
- A clinician will review your eye health before recommending treatment.
- A clinician will review your gallbladder history.
- Because you take insulin or a sulfonylurea, a clinician will review your other medications.
- A clinician will review your digestive history.
- A clinician will review your kidney function.
- A clinician will review your liver function.
- You previously stopped a GLP-1 because of side effects — a clinician will plan a gentler approach.
- A clinician will review the best treatment approach for you.
- A clinician will review your treatment plan in detail.
Coverage
Where we can treat you
Some states require a live video visit before a prescription. We do not yet schedule that visit inside the product — if you live in one of them, write to support@care.plutohealth.com and we will tell you what is currently available before you start. We do not operate a stricter flow in some states and a looser one in others.
States that require a video visit first
Alaska, Alabama, Arkansas, Delaware, Georgia, Hawaii, Idaho, Indiana, Kansas, Louisiana, Missouri, Mississippi, New Mexico, Rhode Island, Texas, Utah. A live video visit with a clinician licensed in your state is required before anything is prescribed, and this product does not yet schedule one for you — contact support@care.plutohealth.com first.
Not served
Puerto Rico, U.S. Virgin Islands, Guam, American Samoa, Northern Mariana Islands. If you live here the questionnaire stops and offers a waitlist. No card details are collected.

What happens once you qualify
A licensed clinician from Pluto Care Medical, P.C. reviews your file within 24 hours. If they decline, you are charged nothing and any amount already taken is refunded automatically.
